The purpose of this report is to provide an annual summary to the Integration Joint Board on the risk management activity and risk registers maintained within the Glasgow City Health & Social Care Partnership during 2023/24.

To provide a summary of the Directions issued by Glasgow City Integration Joint Board (IJB) to Glasgow City Council and NHS Greater Glasgow and Clyde in the period June 2023 to May 2024.

To provide an update on several changes to the membership of the Glasgow City IJB. The report also requests approval of appointments to IJB Committees and notes the remaining vacancies.

This paper proposes a revision to the existing Hospital at Home (H@H) service in Glasgow City and includes the delivery of Glasgow City Call Before You Convey (CBYC), given the alignment of purpose around admission avoidance and synergies in respect of professional and clinical skill sets.
