Be Prepared - Developing Anticipatory Care Planning

In 2015-16, Glasgow City Health and Social Care Partnership used the Integrated Care Fund to develop an Anticipatory Care Plan (ACP) that could be used by people living with a long term condition or conditions that increased the probability of them being admitted to hospital or requiring urgent support.

Glasgow City HSCP Response to HMP Low Moss Inspection Report

David Williams, Chief Officer, Glasgow City Health and Social Care Partnership (HSCP) said: “We are pleased to note that the report acknowledges the work undertaken to address the recommendations made following their 2013 inspection. There has been significant financial investment in this service over the past year.
