Glasgow City HSCP is leading the way in innovative mental health care
Glasgow City Health and Social Care Partnership (HSCP) is leading the way in innovative mental health care, with the successful rollout of a community clozapine titration pathway delivered via the NHSGGC Virtual Hospital – the first model of its kind in Scotland.
This new pathway enables patients requiring clozapine initiation or re-titration to receive care safely in the community, supported by remote monitoring technology, rather than requiring hospital admission. The model has been developed to provide a safe, effective alternative to inpatient care, improving patient experience while reducing pressure on hospital services.
A key factor in the success of this work has been the Glasgow City Community Mental Health Acute Care Service (CMHACS), whose multidisciplinary team has played a central role in both the development and delivery of the pathway. CMHACS clinicians support patient monitoring, medication management, and coordination across services, ensuring a robust and responsive model of care.
The pathway has been delivered at pace and the strong collaboration between CMHACS, pharmacy, community teams, and acute services has enabled rapid implementation and expansion across Glasgow City.
Importantly, the work has already attracted national interest, with other UK services seeking to learn from Glasgow’s approach to delivering clozapine care safely in the community setting. This innovation reflects the commitment of staff across Glasgow City HSCP to redesigning services around patient need - demonstrating how digital health, clinical leadership, and multidisciplinary teamwork can combine to deliver meaningful improvements in care.
A patient said: “Staff were very supportive; they reminded me if I forgot to submit data into the app. The equipment was easy to use. I felt calm doing this at home instead of going into hospital. I do not think I would have opted to try this medication if I could not have done it at home.”
